Score = (EXACTs + 0.5 × NEARs) ÷ total notes × 100,000,000 + shiny EXACTs, rounded down. The timing windows themselves have not been published; the Japanese community wiki notes reports that they may differ by difficulty.

The Miss HP gauge drops by 1 per BREAK and recovers slowly with EXACTs and NEARs. In the v0.2.0 demo it was capped between +13 and -13. Two achievements live right on the line: Borderline Clear (finish on exactly 0) and Borderline Fail (finish on -1).

Score is not Encounter Score

The number above is your accuracy. The separate Encounter Score and its star rating come from damage traded with the opponent, which depends on your cards. The Encounters guide has that formula.

Scoring FAQ

What is the max score in In Falsus?

100,000,000 plus the number of notes in the chart, reached only by hitting every note with the shiny EXACT timing. A 1,500-note chart tops out at 100,001,500.

What score do I need for EX+?

99,000,000. On a 1,000-note chart that is roughly 20 NEARs and no BREAKs, or about 10 BREAKs and nothing else.

Does failing the song stop it early?

No. Running out of Miss HP does not end a song in normal play; you just get Dive Failed at the end. In a Scenario Encounter, though, your Resolve hitting zero ends the Encounter immediately.
